A clearer path for patients with wounds
When a patient receiving dialysis has a concerning or slow-healing wound, our clinic can provide a physician-led wound assessment and coordinate with the existing care team
The wound and the factors around it
Evaluation may include wound history, circulation concerns, swelling, pressure, possible infection, current dressings, and previous treatment. The plan is tailored to the findings and may include coordination with nephrology, vascular care, primary care, or another specialist as appropriate.
Referral notes and recent relevant results can help us understand what has already been tried. Our office can discuss which records are useful and how to send them.
