Consider contacting us when a wound is slow to improve, has changed despite the current plan, or may benefit from assessment of circulation, pressure, swelling, infection, or wound bed needs.
The referring team can share wound history, current orders, relevant test results, and the main question prompting referral. Fax the completed referral form and available relevant records to 810-202-7988, or call with questions about a particular referral.
